This assertive outreach service works within the social model of health and Best Interest Framework to provide a meaningful service to expectant parents, infants, children, and their families to empower them to reach their goals relating to their family, individual, parenting and wellbeing aspects. The IPC health Integrated Family Services Program is a partner in two Orange Door and Integrated Family Services Alliance and the Western Melbourne Catchment area (Cities of Melbourne, Maribyrnong, Moonee Valley, Hobson’s Bay and Wyndham) and the Brimbank Melton Catchment area (Cities of Brimbank and Melton). The Family Services Program team sit across three IPC Health sites: Sunshine, Altona Meadows, and Wyndham Vale.Through a strength-based approach, you will be critical to guiding this group, who may face adversity, in identifying and managing their needs that may impact their developmental stages of life, capacity to self-manage and enable them to build on their strengths and aspirations.The Integrated Family Services team is part of the Innovation and Community Care directorate. It is envisaged that the Family Engagement Practitioner will outreach to identified schools to adopt an immediate, face-to-face approach to promote earlier intervention.The Family Services Practitioner will apply their professional knowledge and skills to engage children, parents, and other significant people. With the family, you will be an enabler and developer of a plan that meets the Best Interest of the Child/ren and the Parent and build on the families’ strengths and aspirations.The Family Services Practitioner will undertake secondary consultation, episodic intervention, group work and short-term work with identified vulnerable families, with warm referral pathways should a longer-term intervention be required.
