We support healthy and empowered lives through innovative models of prevention and care.The OpportunityOur Social Work team within the Allied Health program is seeking experienced Social Worker to join us in delivering high‑quality, client‑centred care to people with complex needs in our community.The opportunity is a Part Time Fixed Term role (0.78FTE) until June 2028 and is classified as Level 5 Pay Point 4 under the Community Health Centre (Stand Alone Services) Social and Community Service Employees Multi Enterprise Agreement 2022-2025.Working across Hobsons Bay and Wyndham, you will play a critical role in supporting individuals, families and carers to build independence, improve wellbeing and navigate the broader health and social service system.You will make a difference by:Receiving referrals and completing comprehensive psychosocial assessments within the context of clients’ family and community environments.Identifying and responding to risk, prioritising workload and managing complex presentations.Developing responsive care plans in partnership with clients, families and carers.Providing high‑quality, holistic Social Work services such as counselling, advocacy, self‑management support, information and referral.Facilitating family meetings, case conferences and stakeholder discussions to ensure coordinated care.Offering secondary consultation to internal and external professionals.Maintaining high‑quality documentation and case records.Planning, delivering and evaluating group programs (subject to capacity and need).Contributing to program planning, service review, and quality improvement initiatives.Upholding the AASW Code of Ethics and organisational values at all times.Meeting service delivery targets and participating in professional supervision. To succeed, you will need:Bachelor of Social Work (or equivalent) and eligibility for AASW membership.Minimum 2 years' experience delivering Social Work services, ideally with complex client presentations.Demonstrated experience working with clients with mental health concerns, comorbidities, cultural complexity, communication barriers and multiple social disadvantage.Expertise in family‑inclusive practice, case conferencing, and systemic approaches to care.Strong understanding of community health principles and the social model of health.Ability to work effectively both autonomously and within multidisciplinary teams.Excellent knowledge of the local health, welfare and community service system.Exceptional communication, negotiation, organisational and time‑management skills.Self‑reliance, innovation and ability to work within organisational policies and protocols.Current Victorian Driver’s Licence, WWCC, and valid Police Check.Eligibility to work with aged care clients (no banning orders).We will offer you:FlexibilityWe work in a flexible, creative and adaptable way to best support our staff and clients.
