The IPC Health Seniors Lifestyle Victoria Program
Team Lead
The opportunity is a Full Time Fixed Term roles from January 2026 until June 2028 and is classified as a Grade 3 under the Victorian Stand Alone Community Health Centres Allied Health Professionals Enterprise Agreement 2022-2026.â¯

The Seniors Lifestyle Victoria program offers access to a range of services to support participants to stay independent and well. This includes support with physical activity, nutrition, medication management, and social connection.
You will make a difference by
ââââSupporting older people to remain active, connected, and independent through the Seniors Lifestyle Victoria pilot program,â¯delivering a timelimited intervention fundedâ¯until June 2028.â¯
ââActing as the primary point of contact within the Seniors Lifestyle Victoria pilot, collaborating with participants, families, and multidisciplinary teams to design and implement programspecific action plans aligned to the pilot’s service model and evaluation framework.â¯
ââEnsuring navigation across health and social care systems for the pilot cohort,â¯facilitatingâ¯access to services specified in theâ¯programâ¯model of care, and empowering participants to actively engage throughout the program.â¯â¯
ââProactively collaborating with internal and external partners to implement, test, and refine the pilot model through rapidcycle quality improvement, datainformed learning, and contribution to formal program evaluation deliverables.ââ¯â
To succeed, you will need:
ââââA tertiary qualification andâ¯at leastâ¯5â¯years’ experience in theâ¯allied healthâ¯or related health field.â¯
ââExperience in effectively leading and managing teams.â¯
ââService and/or process design mapping (an advantage or well-regarded)â¯
ââExcellent holistic clinical assessment and care coordination skills.â¯â¯â¯
ââPassion for supportingâ¯older personsâ¯to build knowledge, skills and confidence toâ¯self-manageâ¯their healthâ¯
ââDemonstrated experience in stakeholder engagement with proven ability to strategically engage, negotiate and influence others to achieve better outcomesâ¯
ââExperience empowering adults in a planned and structured way to improve health and wellbeing outcomes particularly with diverse and disadvantaged communitiesâ¯
ââStrong technical skills withâ¯abilityâ¯to adapt to new platforms and client management systems.â¯â¯
ââCurrent Victorian driver’sâ¯licenceâ¯and Working with Children’s Check.ââ¯â
