Based at our Wyndham Vale and Deer Park campuses, within this role you will be working closely with a multidisciplinary team of health and welfare professionals passionate about the positive impact community-based programs and activities can have on a person’s health and wellbeing; as well as play a pivotal role in the growth and expansion of the Social Prescribing program.As a Wellbeing Coordinator, you will partner with clients to co-create their wellbeing plan and facilitate their social prescription. As a team, you will have the opportunity to liaise with a wide range of community partners to enable engagement as well as build and strengthen local partnerships.We are seeking to recruit 2.6FTE with flexibility for Full Time and Part Time Fixed Term opportunities within this Special Projects role (until October 2021).
