Within this role, you will have a key focus in the provision of casework, short-term counselling and outreach to name a few. Working within a client centred approach and multidisciplinary environment, you will undertake comprehensive assessments to identify current psychosocial support needs and develop a collaborative care plan. This position is a Part Time Fixed Term funding based project role and is classified under the Community Health Centre (Stand Alone Services) Social And Community Service Employees Multi Enterprise Agreement 2017.About YouYou will possess strong case management and care coordination skills and experience with the ability to conduct comprehensive assessments, intervention and individual and group facilitation. You will have proven experience working with and counselling a diverse group of clients experiencing mental health illness or complex needs. With your excellent communication and interpersonal skills, you will be able to effectively promote client empowerment; negotiate and advocate with a variety of stakeholders.
