Liaise with a wide range of community partners to enable engagement as well as build and strengthen local partnerships.Work closely with a multidisciplinary team of Wellbeing Coordinators who aim to connect health service clients to community-based activities to improve their health and wellbeing. Support the team with excellent program management, facilitation and thought leadership to ensure the social prescribing service model, achieves the best possible outcomes.This opportunity is a 0.8FTE Part Time Fixed Term (Special Projects) role until October 2021. Based at our Wyndham Vale and Deer Park campuses, you will also have the opportunity to work from home. This role is classified as an Administrative/Management Officer Grade 5 at $46.99 per hour, under the Victorian Stand-Alone Community Health Services (Health And Allied Services, Managers And Administrative Officers) Multiple Enterprise Agreement 2018-2022.You will make a difference byFacilitating planning, implementation and evaluation processes for Social Prescribing program in accordance to our service agreement and service model.Actively support a culture of continuous quality improvement and best practice frameworks to enhance the service model and achieve the best client outcomes.Building positive relationships with our funders, partners and consumers to continuously improve the social prescribing service model, referral pathways and accessibility of community-based programs for social prescriptions.Provide comprehensive outcome focussed reports and presentations detailing the progress of the program.To succeed, you will needA tertiary qualification in Public Health, Health Promotion or Community development and 4 years’ experience in leading a team-based project delivery setting.Excellent project management skills; including facilitating the broader teams contribution to project deliverables and evaluation outcomes.To be able to strategically engage, negotiate and resolve complex issues with a range of internal and external stakeholders with varying priorities.Analytical skills that can identify operational drivers and the ability to implement improvements to mitigate risk.A special interest in improving social isolation and loneliness; or persistent pain.What next?If you are passionate, creative and want to make a difference, we want to hear from you.
