Providing direct care involves assessing health needs, facilitating referrals, providing information, supporting and developing self management treatment plans alongside clients with chronic conditions. Techniques such as health coaching and motivational interviewing are used to support the client and explore health behaviour change. Acting as a change champion will see you supporting health professionals within the organisation in implementing self management approaches with clients. This opportunity forms part of a multidisciplinary team working directly with clients and will support other health professionals within the service. About YouWe are searching for a motivated allied health clinician who is committed to early intervention in chronic disease management to join our Living Well Program. The successful applicant will require demonstrated knowledge and experience in developing care plans and self management strategies for complex clients with chronic health conditions.
