You will be working closely with eligible client groups encompassing infants and toddlers of young parents, parents with a disability and parents who have recently left State Care. As a Cradle to Kinder Worker, you will be providing a wraparound case management model of support to the primary care giver and family from pregnancy and continues until the child reaches four years old. This role will focus on effective case management of complex needs with the goal of building the parents' or care givers' capacity to self manage during the child's developmental stages.This is an exciting opportunity to join the wider Child and Family services team whilst working as a member of IPC Health to improve and promote community health in the Brimbank region. With your proven experience in the identification, assessment and intervention strategies, you will be able to provide effective case management support to infants with emerging or potential mental health concerns; develop and deliver individual and group work sessions to parents; and provide long –term and short-term early intervention and parental support to families.
